Cost of treatment is certainly a consideration when you and your family need healthcare services. We strive to provide effective treatment within what your finances allow. We accept self-payment and will bill insurances. High deductibles, HSAs, new insurance --- we understand! Regardless, please know that your optimal health and wellness are our foremost concern when we discuss treatment options and make recommendations.
We accept cash, all major credit cards, many private health insurances, Workers' Compensation insurances, and
Motor Vehicle Accident insurances. Copayments and account balances are due at the time of service.
While we can check your insurance benefits, we recommend you also speak with your insurer to review your specific
in-network or out-of-network outpatient physical therapy benefits, limitations, and financial responsibility. You are responsible to know your benefits and how they apply to the services you may receive in our office. We can help determine your estimated out-of-pocket expense for the services you expect to receive. Remember, you are responsible for payment of the service we provide. Your insurance benefits are an agreement between you and your insurance company.
We use nationally standardized codes for the services you receive. We can help with the prior authorization and referral process if needed for insurance reimbursement. We are happy to assist with any other needs.
It is not uncommon for important numbers to change so let us know any time you receive a new insurance card.
If we are in-network with your insurance, we will collect your co-pay at time of service
and bill your insurance for you. Your insurance company will send payment to us, then
we will bill you for any remainder.
If we are out-of-network for your insurance, we offer several options. We can bill your
insurance, then bill you for the balance. We can offer you a 20% discount if you pay in full
at the the time of service and you get reimbursed by your insurer. Or, we can offer a
10% discount if you pay in full at the time of service and we submit the bill your insurer
for you to be reimbursed. We can provide estimates so that you can decide which option
is best for you!
We are currently in-network with: BCBS, Lifewise, MHN, SAIF, CareMark Comp, and more. We must bill an
insurance carrier and cannot offer discounts when we are in-network, or if yours is a Workers' Compensation
or MVA claim.

No Show/Late Cancellation Policy
Our cancellation policy requires a minimum of 24-hour notice prior to your scheduled appointment. Attending consistently helps Kristi progress your treatment program effectively. Conversely, not cancelling early prevents another patient from receiving treatment. In cases with sufficient notice, we are able to offer the appointment to those on the waiting list. If you cancel less than 24 hours prior to the appointment, and we are unable to fill the opening, be prepared to pay a $50 fee for the missed appointment. Not showing for an appointment is the same as a late cancelation; you will need to pay the full $50. Of course, there are allowances for extenuating circumstances. Please also refer to our
