Cost of treatment is certainly a consideration when you and your family need healthcare services. We strive to provide effective treatment within what your finances allow. We accept self-payment and will bill Workers' Compensation and MVA insurances. High deductibles, HSAs, new insurance --- we understand! Regardless, please know that your optimal health and wellness are our foremost concern when we discuss treatment options and make recommendations.
Your payment is due at the time of service. We accept cash, checks, all major credit cards, Workers' Compensation insurances, and Motor Vehicle Accident insurances.
Payment for Your Treatment
We bill the insurance carrier for you at our full rates if yours is a Workers' Compensation or MVA claim.
All other patients pay in full at the the time of service at a 20% discount. If you will be seeking reimbursement from your insurer, you may send in the claim form which we provide to you.
We can offer you a 10% discount if you pay in full at the time of service and request that we submit the forms to your insurer.

Insurance Services
While we can check your insurance benefits, we recommend you speak directly with your insurer to review your specific
out-of-network outpatient physical therapy benefits, limitations, and financial responsibility. You are responsible to know your benefits and how they apply to the services you may receive in our office. We can help determine your estimated out-of-pocket expense for the services you expect to receive.
Remember, you are responsible for payment of the service we provide. Your insurance benefits are an agreement between you and your insurance company.
It is not uncommon for important numbers to change so let us know any time you receive a new insurance card.
No Show/Late Cancellation Policy
Our cancellation policy requires a minimum of 24-hour notice prior to your scheduled appointment. Attending consistently helps Kristi progress your treatment program effectively. Conversely, not cancelling early prevents another patient from receiving treatment. In cases with sufficient notice, we are able to offer the appointment to those on the waiting list. If you cancel less than 24 hours prior to the appointment, and we are unable to fill the opening, be prepared to pay a $50 fee for the missed appointment. Not showing for an appointment is the same as a late cancellation; you will need to pay the full $50. Of course, there are allowances for extenuating circumstances. Please also refer to our
